On-Line Process is two steps:

STEP 1

  • Select Proof of Claim (must be submitted on or before September 23, 2019
  • Go to bottom of screen and open, complete, save, print and scan the Proof of Claim pdf
  • Select SETTLEMENT FUND and then select APPLY
  • Complete Contact Information and then select CONTINUE
  • Select method to receive a Verification/Confirmation Code and then select SUBMIT
  • Enter Code and select VERIFY
  • Follow instructions on Applying for Settlement Fund screen
  • Scroll down and at the 3rd Gray Box at "Upload Documents," upload Proof of Claim Form and all supporting documents, if applicable (e.g., Power of Attorney, Letter of Administration, etc.)
    • IMPORTANT NOTE: If you miss uploading the Proof of Claim at this step, then YOU MUST email the Proof of Claims to:  claims@cbrownassoc.com
  • Answer the 4 questions on the next screen
  • Review your application, then select NEXT
  • Read Self Attest, check, sign and select SUBMIT
  • STOP - Thank you!  We have received your application
  • Go to the left-hand margin and select Sign out

After you complete Step 1, you will receive an email confirmation from the Claims Administrator directing you to complete Step 2, the final step of the on-line registration process.

STEP 2

  • Follow the instructions on the screen to submit all remaining required information
  • Once you have completed Step 2, you will receive a second email from the Claims Administrator either confirming your submission is complete or requesting additional information
  • FYI and prepared to complete the following forms during Step 2 of the on-line process:
  • Note: If you register on-line, upon completion of the registration process, you will be able to review, update, edit or change your profile.

Mailing InstructionsTo file a Proof of Claim by mail, complete the following and include supporting documents, if applicable (e.g., Power of Attorney, Affidavits, etc.):

Please read the instructions carefully when completing the Proof of Claim.  Include all documents required above.  Mail the completed Proof of Claim and all required documents postmarked on or before September 23, 2019 to: Claims Administrator, AFGE Local 3313, c/o C. Brown & Associates, Inc., 9646 Pennsylvania Avenue, Upper Marlboro, MD 20772.

Once the Claims Administrator has received your Proof of Claim and all required and supporting documents, you will receive a confirmation email/letter and/or a request for additional information.

NO PAPER CHECKS WILL BE DISTRIBUTED 

Authorized Claimants will receive payment from the Settlement Fund either by direct deposit or Wisely Pay card.
